The Office Solutions Benefit Program offers you a variety of medical plan choices. Depending on your individual needs, you may select one of these four plans. Feel free to click on each of the plans to pull up benefit summaries.

In order to view the per-pay-period deduction amounts for the medical plans, please click on the 2020 Payroll Authorization Deduction Form on the left. If you choose to enroll in any of the plans, please print, complete and sign the form and submit to Human Resources.

Annual Report for Benefit Plan Participants
All Employees who were OS benefit Plan Participants for 2016 – 2017:
Re: Summary Annual Reports for the OS Medical, Dental, Vision & Life benefit plans held from 2/1/2016 through 1/31/2017. Each year, we are required to provide all plan participants with the Summary Annual Report for any medical benefit plans held and offered by the company from the prior year. The Summary Annual Report provides you with basic financial data for those benefits plans offered by OS, in a format required by the United States Department of Labor and provide instructions on how you may obtain additional information if needed, about the plan. Required Notice: The attached report is the Summary Annual Report for all benefit plans held by OS for the year ending 1/31/2017. No Action is Required: This report is for your information only; you do not need to do anything, other than retaining this report with your benefits information with Office Solutions.
